A Steady Guide When the Path Feels Unclear

 Caregiving often begins without warning.
A diagnosis. A concern that won’t go away. A moment when something no longer feels right.


Families come to CarePath not because they lack love or commitment — but because they’re trying to make thoughtful decisions in the middle of uncertainty, emotion, and urgency.


CarePath Family Navigation exists to help families slow things down, understand what’s happening, and move forward with clarity and confidence.

Who I Am

 I’m Elayne Forgie, founder of CarePath Family Navigation.


For more than 30 years, I’ve worked alongside families navigating aging, dementia, and complex care decisions — from the first concerns through later-stage and end-of-life care.


I’ve supported families privately and in group settings, helped them evaluate options, plan transitions, and manage the emotional weight that caregiving places on the entire family.


CarePath was created to provide the kind of guidance families often wish they had earlier — before decisions feel rushed or overwhelming.

How I Work

 I don’t replace medical providers, attorneys, or community agencies.


I help families make sense of the information, weigh options thoughtfully, and decide what makes the most sense for their situation.


My role is to:

  • Help you understand what’s happening and what it may mean 
  • Clarify priorities when opinions differ or emotions run high 
  • Anticipate decisions before they become urgent 
  • Support family communication and alignment 
  • Walk with you through care transitions and next steps
     

This work is not about having all the answers.
It’s about helping families make informed decisions — one step at a time.
