I’m Elayne Forgie, founder of CarePath Family Navigation.
For more than 30 years, I’ve worked alongside families navigating aging, dementia, and complex care decisions — from the first concerns through later-stage and end-of-life care.
I’ve supported families privately and in group settings, helped them evaluate options, plan transitions, and manage the emotional weight that caregiving places on the entire family.
CarePath was created to provide the kind of guidance families often wish they had earlier — before decisions feel rushed or overwhelming.
I don’t replace medical providers, attorneys, or community agencies.
I help families make sense of the information, weigh options thoughtfully, and decide what makes the most sense for their situation.
