Experienced Guidance for Families Facing Dementia and Comple

 I’m Elayne Forgie, founder of CarePath Family Navigation. For more than 30 years, I’ve helped families navigate the changes involved in caring for someone with dementia and other aging challenges.

Experience & Leadership

 My experience is rooted in hands-on leadership and long-term work with families navigating dementia and aging-related care.

Over the course of my career, I have:

  • Founded and led dementia-focused home care and care management organizations, developing services designed to support families across multiple stages of caregiving
     
  • Designed and implemented memory care and Alzheimer’s-focused programs, working closely with individuals living with cognitive change and those caring for them
     
  • Supported families through diagnosis, behavioral changes, caregiver stress, family dynamics, care transitions, placement decisions, and end-of-life planning


  • Served in advisory and board-level roles with the Area Agency on Aging Palm Beach/Treasure Coast and the Alzheimer’s Association SE Florida, supporting education, advocacy, and community-based initiatives focused on aging, dementia, and caregiver support.

     

These experiences shaped how I guide families today — with perspective, steadiness, and a deep respect for the complexity of caregiving.

My Approach to Navigation

 My work as a Family Care Navigator is grounded in long-term, real-world experience walking alongside families through every stage of the caregiving journey.


Families come to me when they are noticing changes, facing difficult decisions, or trying to make sense of what comes next. 

My role is to help them slow things down, think clearly, and move forward with greater confidence and perspective.


This work is not about quick answers — it’s about helping families understand their situation and make decisions they can feel steady about over time.

How this helps families

 Caring for someone with dementia or age-related needs often brings uncertainty, conflicting information, and emotional strain.


Because I’ve spent decades working directly with families in these situations, I help families:


  • Understand what’s changing and what it may mean
     
  • Sort through options without feeling rushed or pressured
     
  • Anticipate decisions before they become urgent
     
  • Communicate more effectively with care providers and family members
     
  • Feel more grounded and confident about next steps
     

This work is about clarity, steadiness, and thoughtful navigation — not quick fixes.
