By accessing this website or using CarePath Family Navigation services, you agree to the following Terms & Conditions.

CarePath Family Navigation provides care navigation, guidance, and decision support for individuals and families navigating dementia and complex caregiving situations.

CarePath services are:

• Educational and supportive in nature 
• Not medical advice 
• Not therapy or counseling 
• Not legal or financial advice
   

CarePath does not diagnose medical conditions, prescribe treatment, or replace licensed healthcare, legal, or mental health professionals.

All information provided on this website is for general informational purposes only. Decisions regarding medical care, legal matters, or treatment should be made in consultation with appropriate licensed professionals.

The CarePath Clarity Session is a paid, time-reserved service.

• Payment is required at the time of booking 
• Sessions are conducted virtually 
• Missed sessions or late cancellations may not be refundable (see Cancellation Policy)
   

Because session time is reserved specifically for you:

• Cancellations or rescheduling must be made at least 24 hours in advance 
• Sessions canceled with less than 24 hours’ notice may be forfeited 
• No-shows are not refundable
   

Exceptions may be made at CarePath’s discretion for emergencies.

CarePath Family Navigation is not responsible for outcomes related to decisions made by clients or families. By engaging CarePath services, you acknowledge that all decisions remain your responsibility.

CarePath’s liability is limited to the amount paid for the service provided.

All content on this website, including text, logos, and materials, is the property of CarePath Family Navigation and may not be reproduced without written permission.

CarePath Family Navigation reserves the right to update these Terms & Conditions at any time. Continued use of the website or services constitutes acceptance of any changes.
